Evaluation of adults. Basic measurements of weight, height, Body Mass Index, waist circumference, blood
pressure, screening exam of head, lungs, heart, abdomen and extremities as well as complaint driven focused exams
were performed. Results of each examinations were documented and further analyzed through Excel program. Averages
were counted.
In summary, we have screened close to 300 adults, but complete measurement data were available only on 115 as there
was a lack of scale on two consecutive days.
Average weight was 51 kilograms and average height was 159 cm.
Average BMI (Body Mass Index) was 20.625.
There were 27 people ( 27.5%) of screen were underweight ( BMI <18.5) The lowest BMI measured was 15.
The abdominal waist circumference on average was 28 inches or 73 cm.
The average BP (blood pressure) was 121 and diastolic was 76.
Most common complaints:
Men: lower back pain, followed by cough, tootache, headache, rash, joint aches. Also complaints of penile discharge,
hematuria, fatigue and dizziness were common.
Women: lower abdominal discomfort, vaginal discharge, fatigue and dizziness were most common. Less common were
headaches, rash, tootache, insomnia.
Only one person exhibited signs and symptoms of depression.
The most staggering was the frequency of poor dentition which was observed almost in all screened patients.
The second most staggering fact was the lack of seeing people over age 50.
Only few survivors over age 50 were seen, mostly women and a few men.
General assessment
consisted of measuring weight, height, screening
physical exam, complaint driven focused exam and
treatment based on clinical findings.
We have collected complete set of data on
approximately 115 adults and partial set of data on close
to 300 adults.
Nutrition
Our findings indicate that almost all people have BMI
less than 25. The average BMI was 20.3 and
approximately 27% of people were underweight/
malnourished ( BMI < 18.5).
Only one person was overweight.
The abdominal waist circumference was low, with
resulting average of 28 inches/ 73 cm.
Wounds &wound care
Many people presented with either untreated or poorly
self-cared wound infections.
Local staff was trained on basics of wound care and use of
appropriate supplies and techniques.
Poor dentition
The unexpected findings of very poor dentition among
most otherwise healthy adults was striking.
Many adults reported no toothbrushing primarily due to
cost.
Cardiovascular assessment
the unexpected findings of significant lack of hypertension in
general population below age 50. This only confirmed the
existing relationship between BMI and prevalence of high
blood pressure.
Some adults ( few women in early 40's) and a few men as
well as women over age 50 were found to have undiagnosed
and untreated sometimes severe hypertension. There were
no findings of diabetes.
Cholesterol levels were not tested.
Heart murmurs or complaints of shortness of breath were
infrequent.
Women's health
None of the pregnant women had access to prenatal
supplements. In the villages prenatal care was minimal to
none.
In women ( both pregnant and non-pregnant) lower
abdominal pain and vaginal discharge were the most
common complaints.
Average age of first child was at or around age 15.
There was no routine gynecological care for the village
women available. Nearest hospital was more than 15 miles
of walking distance.
Malaria & TB & HIV
We still found lack of preventive mosquito nets, inadequate
therapy in a chloroquine resistant malaria region, and high
frequency of reported repetitive malaria infections. We
were very pleased to see that a new regional initiative for
proper drug resistant malaria was underway.
No one has reported a family member who died of
tuberculosis, but the feel for prevalence could not be
established.
HIV test is at present preformed at Anivorano Hospital on
every pregnant woman. No other screening was at this
time available. Only one HIV positive case was so far
reported.
Other findings
While malnutrition, low BMI, poor dentition, genital
infections and complaints of lower back pain dominated the
screening, few patients presented with:
end stage liver disease, chronic hematuria, large
abdominal masses and or untreated hernias.
Skin rashes were thought to be predominantly due to fungi.
